What format can be converted to midi without crappy results?

Simple answer.. none. MIDI's are primarily done by hand.

I've tried a few wave to midi proggys, almost as quick and much better result to program them yourself. There's a few programs that make programming midi easy, like band in a box, or jammer, but to get it precise, 'note by note' is the way to go

Have you tried digital ear?? Converts wave to midi. The results depend on the monophonic instrument your using. (it can sound like you've gone daft with the mod. and pitch wheel!) Try the demo first.
